JOB PURPOSE:
The Health Services Manager oversees all City on a Hill health services, which include a medical clinic, an integrated dental hygiene program, a behavioral health clinic, and health promotion and education. In addition to oversight of the 5 day a week health services the manager also oversees a monthly Saturday Community Health Outreach event which includes access to health services. This role oversees the operation and quality improvement program of the health services, helping to ensure that it accomplishes its objectives by measuring and reporting outcomes and supervising staff. The Health Services Manager is integral to community health, patient and client treatment, education, follow-up, clinical data gathering, grants management, and working with numerous community and university partners.
This position oversees primary care coordination for uninsured patients in the Free Clinic and Medical Homes programming with partners, medical volunteers, and staff members. This position has a lead role in implementing grant-funded programs emphasizing health prevention education and services, health screenings, and assisting patients navigating the healthcare system. If an individual with a clinical background is hired this individual will provide clinical care appropriate to their licensure.
ESSENTIAL DUTIES & RESPONSIBILITIES:
1. Health Services Oversight (60% of role)
• Oversees clinical operations of a free health clinic open up to five days per week, providing medical care and experience to all patients and clients.
• Works with the Medical Director to establish and maintain Standards of Care, health policies, and medical guidelines for all health services, including provider visits, and maintains all needed medical record forms, prescription forms, medication labels, and provider visit logs.
• Coordinates quality improvement program and works with the Medical Director to achieve and maintain WAFCC Standards of Excellence which provide direction for goal setting, planning to meet goals, evaluating plans, and for guiding projects and actions.
• Oversees and manages monthly community Health Outreach events, working with other health staff, leading volunteers, and other health agencies. Oversees all medical areas utilizing lead volunteers in each medical area and coordinates activities with other staff members in leadership roles
• Oversees Medical Homes programming in conjunction with the Medical Director, health staff, clinical students, and health volunteers to recruit community partner sites and to screen, educate, and engage participants in the program.
• Provides oversight and guidance to health staff to coordinate behavioral health services in partnership with Anesis Therapy, Wisconsin Association of Free and Charitable Clinics, Nurse Disrupted, including individual crisis and non-crisis teletherapy sessions.
• Serves as the lead superuser of the electronic health record (EHR), Athena and as primary point of contact daily for users of the EHR
• Works with the Medical Director and Development Team in preparing, reporting, and implementing new grant-funded programs within the Health Services department.
• Collaborates with the health team to maintain the COAH Health Application for clinic patients and the community.
• Works with the Volunteer Coordinator to recruit, screen, and onboard new health volunteers. Develops volunteer team and communicates with lead medical volunteers to determine schedule availability to meet upcoming clinical needs.
• Works to maintain an efficient and effective Intake Process and maintains health clinic forms and documents on the server to ensure proper forms are being used and accurate data is recorded.
• Creates and maintains proper guidelines for preventative health screenings and services offered to prevent chronic illnesses and disease.
• Works with the Medical Director to negotiate annual contracted lab rates/codes with LabCorp Lab Services according to clinic needs and capacity.
• Track qualitative and quantitative data and guests’ progress for monthly reports and future success stories.
• Gather guest feedback and adjust programming based on experience and program evaluation.
• Identifies, researches, and solves problems in a timely manner and implements strategies to avoid similar problems in the future.
• Implements processes and systems that improve efficiency and effectiveness.
2. Leadership Role Responsibilities (20% of role)
• Provide supervision, coaching and regularly meets with direct reports, completes annual performance reviews.
• Develops 4th Quarter projections and annual budgets (income & expense) for designated Budget account projects.
• Approves invoices and manages expenditures in designated budget accounts throughout the year.
• Completes short-term and long-term planning for the health area, improves the excellence of the departments, and sets a sustainable pace for subordinates.
• Proactively provides relevant and timely information to other staff to reduce confusion and increase efficiency.
• Effectively delegates and coordinates the work of subordinates and volunteers, promoting team effort.
• Attends monthly Leadership meetings and scheduled Leadership Day Aways, actively participates in dialogue and training.
• Assists with the development and reporting of content for grant applications.
• Implements grant-funded programs and activities and ensures proper data collection for all departmental activities/programs.
3. Partnerships & Community Collaborations (10% of role)
• Partners with local universities and colleges to provide mutually beneficial medical experiences for students and colleges and develops relationships/partnerships with other local universities and colleges as opportunities arise.
• Works with the Medical Director to screen requests from medical/PA/NP students for volunteer opportunities in the Health Clinic.
• Communicates and community networks with other Free Clinics and healthcare organizations in the Milwaukee area, represents COAH externally in city/state-wide collaboration.
• Attends monthly FC3 (Free Clinic Collaborative) and WAFCC (WI Assoc of Free & Charitable) meetings as a City on a Hill representative and utilizes resources as needed.
• Updates Health data on community databases: City of Milwaukee, Milwaukee Medical Society, Badger, 211, Unite Us, Safety Net, etc.
4. Health Education & Navigation Support (10% of role)
• Provides oversight of the Community Health Worker and other clinic staff to connect patients to utilizing the health care system, finding a medical home, and providing referrals to health and social services.
• Provides informal counseling and advocacy for individuals and families to increase knowledge and self-sufficiency in large, small groups, and 1:1 settings.
• Supports the Community Health Worker and other clinic staff with individuals through the insurance enrollment and navigation process and Badger Care/Medicaid enrollment process.
• Attends necessary training to stay updated with program best practices, services, and community resources that will benefit those we serve.
• Provides oversight to the health team in developing relevant annual health education
emphasis calendar that utilizes university partnerships and volunteers to develop displays, handouts, and staffing coordination.
• Utilizes volunteer nurses and medial and health students in teaching monthly educational topics to fulfill grant requirements and meet patients needs.
• Collaborates with FC3 Social Worker to assist guests with complex needs and issues.
Complete other activities and task as assigned.
Adheres to the values of City on a Hill and contributes to the unity of the team.
Attends all-staff meetings, initiatives, and celebrations and collaborates with staff on discipleship for City on a Hill’s strategic plan.
MINIMUM QUALIFICATIONS:
SPECIALIZED KNOW-HOW AND REQUIREMENTS:
• Registered Nurse or other health professional including but not limited to healthcare administrator, public health professional, health promotion professional, mental health professional with broad experience working in underserved communities.
• Minimum five years experience in an outpatient clinic, public health system, hospital setting.
• Minimum three years experience managing operations and personnel.
• Experience with a wide range of preventative health education topics with the ability to teach in 1:1, small group, or large group and community settings.
